Pathways Healthcare in Londonderry, NH is seeking a licensed Occupational Therapist to join our in-home care team. You will conduct comprehensive patient assessments, develop individualized treatment plans, and train patients and families in daily activities and use of adaptive equipment.

This full-time role includes a competitive salary of $80,000–$90,000, a robust benefits package, three weeks of PTO to start, and opportunities for professional growth.

Qualifications

• Hold an occupational therapy degree accredited by AMA/AOTA and be registered with the AOTA's National Registration Exam.
• Minimum two years of clinical OT experience preferred.
• Strong verbal and written communication and organizational abilities.
• Current BLS CPR certification.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Conduct comprehensive patient assessments including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.

Position Details:

• Service Area: Derry, Londonderry, Merrimack, Litchfield, Hudson areas.
• Job Type: Full Time; Per Diem applicants are welcome to apply.
• Schedule: Monday-Friday

Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Maintain a current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

• Salary Range: $80,000 - $90,000
