Job summary

Pathways Healthcare in Massachusetts seeks a licensed Occupational Therapist to provide in-home and hospice rehabilitation services. The role includes evaluating patients, planning individualized treatments, and training families for continued care.

Qualifications

• Degree from OT curriculum accredited by CAAHEA and AOTA and registered with AOTA.
• Minimum of two years of clinical occupational therapy experience preferred.
• Valid driver's license and own vehicle for home-based visits.
• Excellent verbal and written communication with strong organizational abilities.

Responsibilities

• Conduct comprehensive patient assessments including functional status and home environment.
• Develop and implement individualized OT treatment plans with measurable goals.
• Train patients in activities of daily living, prosthetics, orthotics, and adaptive equipment use.
• Educate families and caregivers on treatment plans and progress.
• Maintain accurate records and report condition changes promptly.
• Supervise occupational therapy assistants and home health aides.
• Coordinate discharge planning and refer to additional services as needed.
• Participate in trainings, meetings, and case reviews.

Responsibilities:

• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.

• Refer patients to additional services as needed and coordinate discharge planning.

Qualifications:

• Hold a degree from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Possess a valid driver's license and own a reliable, insured vehicle.
• Salary Range: $80,000 - $90,000
