Job summary

Pathways Healthcare is a leading home health and hospice organization focused on patient-centered care. We are seeking an Occupational Therapist to conduct comprehensive assessments, develop personalized plans, and educate families for in-home recovery.

Ideal candidates have an accredited OT degree, active licensure, and 2+ years of clinical OT experience, with strong communication and organizational skills.

Qualifications

• Accredited OT degree and licensure via national exam.
• Two years of clinical OT experience preferred.
• Excellent verbal and written communication with strong organizational abilities.

Responsibilities

• Conduct comprehensive patient assessments, including functional status and home environment.
• Evaluate work capacity and arrange vocational training referrals.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services and coordinate discharge planning.

Position Details:

• Main Office Location: Concord, MA
• Service Area: Billerica, Bedford, Lowell, Chelmsford, Westford areas.
• Job Type: Full Time
• Schedule: Monday-Friday

Qualifications:

• Hold a degree from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Maintain a current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Our Benefits:

• Salary Range: $80,000 - $90,000
