Job summary

Pathways Healthcare, Llc is seeking an Occupational Therapist to join our physician- and nurse practitioner-led home health and hospice team in Concord, MA. You will conduct comprehensive in-home assessments, develop personalized treatment plans, and train patients in daily activities and adaptive equipment.

Qualifications

• Hold an OT degree accredited by the AMA/ACHA and AOTA or equivalent.
• Registered with the National Registration Examination of the AOTA.

Responsibilities

• Conduct comprehensive patient assessments including functional status and home environment.
• Evaluate work capacity and provide vocational training referrals as needed.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Coordinate discharge planning and referrals to additional services.

• Refer patients to additional services as needed and coordinate discharge planning.

Qualifications:

• Hold a degree from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Possess a valid driver's license and own a reliable, insured vehicle.
• Salary Range: $80,000 - $90,000

• Three weeks of PTO during first two years, increase to four weeks after two years of being benefits eligible.
