Job summary

Domnovate is seeking an occupational therapist to support home-health patients across Alameda County in Oakland, CA. In a 13-week W-2 contract, you will evaluate daily living needs, develop practical therapy plans, and supervise assistants in patients' homes, with travel between visits.

You will work Monday–Friday, 40 hours per week, starting late October 2026, with three days of initial training. A California OT license and BLS are required, along with strong documentation and communication

Qualifications

• Active California occupational therapist license.
• At least one year of home-health experience.
• Current BLS certification.
• Strong assessment, care-planning and communication skills.
• Ability to prioritize start-of-care visits, supervise assistants and travel between homes.

Responsibilities

• Provide direct occupational therapy care in patients' homes across Alameda County.
• Develop and implement practical therapy plans to improve daily living independence.
• Coordinate with caregivers and supervise occupational therapy assistants.
• Document timely clinical notes and maintain care plans.
• Travel frequently between patients' homes using provided tools.

Job description

Total Compensation: $36.26–$38.53/hour*

Help patients build independence where it matters most: in their own homes. Evaluate daily living needs, develop practical therapy plans and help patients and caregivers turn treatment goals into everyday progress. This Alameda County assignment combines direct occupational therapy care with collaboration, timely care coordination and supervision of occupational therapy assistants. An iPad and communication tools are provided to support your work in the field.

Assignment Overview

• Oakland, California 94621; home-health visits throughout Alameda County, with territory expansion as needed.
• 13-week W-2 contract; 1 opening
• Monday-Friday days, 40 hours per week.
• Proposed start: October 28, 2026, pending facility confirmation.
• Three full days of initial training, including office and field training.
• Required business mileage, parking and tolls are reimbursed separately under assignment terms.

What You Bring

• Active California occupational therapist license and at least one year of home-health experience.
• Current BLS certification.
• Strong assessment, care-planning and communication skills, including timely clinical documentation.
• Ability to prioritize start-of-care visits, supervise occupational therapy assistants and travel between patients’ homes.

Pay and Benefits

Taxable W-2 hourly pay: $25.00–$27.27.

For eligible travelers, total compensation may include both taxable W-2 wages and separate travel reimbursements under Domnovate Healthcare’s travel reimbursement policy.
