Choice Matters Home Healthcare in Jacksonville, FL is seeking a Home Health Registered Nurse (RN) to perform comprehensive nursing assessments, develop individualized plans of care, and provide skilled nursing care in patients' homes.

You’ll collaborate with physicians, therapists, social workers, and other team members, monitor patient progress, and maintain Medicare and agency standards, using AI-assisted documentation tools and WellSky EMR to streamline charting while ensuring high-quality

Qualifications

• Current Florida Registered Nurse (RN) license.
• Valid driver’s license and reliable transportation.
• Excellent clinical, communication, and organizational skills.
• Home health experience required.
• OASIS experience required.

Responsibilities

• Perform comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing care, patient education, and care coordination in the home.
• Collaborate with physicians, therapists, medical social workers, and other members of the care team.
• Monitor patient progress and communicate changes in condition promptly.
• Complete accurate and timely documentation while maintaining Medicare and agency standards.
