Job summary

Cascadia EE LLC in Oregon seeks an Occupational Therapist to assess referred patients, develop rehab plans, and deliver direct care in a long-term care setting, evaluating medical conditions, functional capacity, and rehabilitation potential under physician direction.

You will document progress, educate patients and families, supervise COTA(s) and rehab aides, collaborate with the interdisciplinary team, ensure punctual shifts, and contribute to care planning and OBRA-compliant practices.

Qualifications

• Graduated from an accredited school of occupational therapy.
• Current state license as an Occupational Therapist.
• Current CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Assess referred patients and establish treatment plans with goals.
• Document progress and communicate with patients, families, and staff.
• Supervise COTA and Rehab Aide in therapy sessions.
• Contribute to facility care planning and coordination.
• Ensure punctuality and attendance for shifts.
