Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking a talented Occupational Therapist to join our growing rehabilitation team. You will assess referred patients, provide direct care, and develop treatment plans aligned with physician direction.

You will supervise Occupational Therapy Assistants, document progress, educate patients and families, and collaborate with all disciplines to deliver patient-centered care while adhering to confidentiality and HIPAA guidelines.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service and clinical excellence.
• Timely attendance and professionalism.
• Perform duties as assigned by Director of Rehabilitation.
• Confidentiality, HIPAA and Residents Rights compliance.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop treatment programs with goals based on patient capacity.
• Document patient condition and educate patients and families.
• Supervise Occupational Therapy Assistants.
• Ensure confidentiality and HIPAA compliance.
• Coordinate with other disciplines for patient-centered care.
