Job summary

Sierra Valley Rehab Center in Porterville, CA is actively seeking a licensed Occupational Therapist (OT) to join our therapy team in a post-acute care setting. You will evaluate, plan, and provide therapeutic interventions to help residents regain independence in daily activities.

This role involves collaborating with an interdisciplinary team, supervising COTAs, educating residents and families, and contributing to discharge planning. CA licensure and CPR are required or obtained upon hire.

Qualifications

• Licensed Occupational Therapist with current CA license.
• Experience in skilled nursing or post-acute care preferred.
• Proficient with EMR Casamba or PointClickCare.
• CPR certification or to be obtained upon hire.
• Ability to work in a team and communicate with residents, families, and interdisciplinary staff.

Responsibilities

• Evaluate patients within 24 hours of physician referral or document cause for delay.
• Develop and implement individualized treatment plans.
• Provide direct therapy services and document outcomes.
• Supervise COTAs and aides per state guidelines.
• Educate residents, families, and caregivers on therapy goals and home programs.
• Participate in discharge planning, staff training, and in-service programs.
• Attend care conferences and communicate patient progress to the interdisciplinary team.
• Recommend adaptive equipment and modifications to support functional independence.
