Job summary

Sierra Valley Rehab Center in Porterville, CA is a skilled nursing and rehabilitation facility seeking a licensed Occupational Therapist to join our therapy team.

You will evaluate, plan, and provide therapeutic interventions to help residents perform daily activities, working with interdisciplinary teams in a post-acute care setting. We value compassionate care, strong clinical skills, and the ability to educate residents and families.

Qualifications

• Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program.
• Current California OT license in good standing.
• CPR certification (or obtained upon hire).
• Experience in skilled nursing or post-acute care preferred.
• Proficiency in EMR systems such as Casamba or PointClickCare.
• Strong clinical, organizational, and communication skills.

Responsibilities

• Evaluate patients within 24 hours of physician referral or document cause for delay.
• Develop and implement individualized treatment plans.
• Provide direct therapy services and document outcomes in accordance with facility, state, and federal requirements.
• Supervise Occupational Therapy Assistants (COTAs) and aides as per state guidelines.
• Educate residents, families, and caregivers on therapy goals and home programs.
• Participate in discharge planning, staff training, and in-service programs.
• Attend care conferences and communicate patient progress to the interdisciplinary team.
• Recommend adaptive equipment and modifications to support functional independence.
• Maintain therapy equipment and report any maintenance needs.
