Job summary

Gateway Post Acute in Porterville, CA is seeking a PRN Occupational Therapist to join our rehabilitation team. The OT evaluates and treats patients, develops individualized plans, and collaborates with an interdisciplinary team to promote functional independence.

This role offers flexible PRN scheduling, competitive pay of $60-$65 per hour, and opportunities to support patient recovery in a skilled nursing and post-acute setting.

Qualifications

• Bachelor's degree in Occupational Therapy is required.
• Master's or Doctorate degree is preferred.
• Experience in Skilled Nursing Facility or post-acute care is a plus.

Responsibilities

• Evaluate patients and establish individualized OT treatment plans.
• Treat patients per physician orders and plans of care.
• Document evaluations, daily notes, progress notes, and discharge summaries per regulations.
• Educate patients and families on adaptive techniques and safety.
• Collaborate with nursing and interdisciplinary teams for optimal outcomes.
• Participate in care conferences and discharge planning.
