Job summary

Hertford Rehabilitation and Healthcare Center, a Long Term Care facility in Hertford, NC, seeks a dedicated Occupational Therapist to join our growing team. You will perform comprehensive OT evaluations, develop treatment plans, and guide patients toward independence.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.

Responsibilities

• Provide comprehensive occupational therapy evaluations based on MD orders.
• Document findings on standardized evaluation forms timely, accurately, and adhering to all guidelines.
• Develop and implement individualized treatment plans, including long and short-term goals, frequency, duration, and treatment modalities.
• Provide occupational therapy interventions to patients, utilizing various modalities and modifying treatment as indicated.
• Document patient participation and progress on progress notes accurately and timely.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Participate in facility and rehab meetings as indicated.
• Maintain accurate and complete records of patient care and services rendered.
