Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is seeking an Occupational Therapist (OT) for full-time, part-time, or PRN roles. You will evaluate residents, develop treatment plans, document progress, and collaborate with a dedicated rehab team to deliver high-quality care.

Qualifications

• Graduate of an accredited OT program with state license.
• SNF experience preferred.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Provide a comprehensive treatment plan including goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, and discharges.
• Deliver comprehensive treatment using appropriate modalities and modify as needed.
• Document daily participation and progress and update plans and discharge summaries.
• Attend facility meetings and participate in committees as indicated.
• Supervise junior staff, students and technicians as needed.
