Job summary

Hertford Rehabilitation and Healthcare is seeking a dedicated Occupational Therapist to join our rehab team in Hertford, NC. The role offers full-time, part-time, or PRN options with a strong emphasis on comprehensive OT evaluations and individualized treatment planning.

You will work within an interdisciplinary team, document progress, and ensure high-quality patient care while maintaining compliance with policies and licensing requirements.

Qualifications

• Graduate of an accredited university with an OT degree (B.S., M.S., or Doctorate).
• SNF experience preferred.
• Current OT license/registration in NC as applicable.

Responsibilities

• Provide comprehensive OT evaluations based on MD orders.
• Develop treatment plans with long- and short-term goals, frequency, duration, and modalities.
• Document treatments, progress, and discharge summaries accurately and timely.
• Coordinate with MDs, nurses, and rehab staff in an interdisciplinary team.
• Supervise junior staff and students as needed; co-sign notes where required.
• Maintain patient confidentiality and comply with facility policies and regulatory guidelines.
• Assist with billing and documentation for occupational therapy services.
