Job summary

Waverly Rehabilitation And Healthcare Center in Waverly, VA is seeking a full-time Occupational Therapist to join our Long Term Care team. You will perform comprehensive OT evaluations, develop and implement treatment plans, and collaborate with an interdisciplinary team to support residents.

The role requires an accredited OT degree, VA licensure, and preferred SNF experience. You will document progress, manage care plans, and participate in facility meetings.

Qualifications

• Graduate of an accredited OT program recognized by the AOTA.
• Current license/registration in the state (as applicable).
• Experience in SNF settings preferred and demonstrated documentation skills.

Responsibilities

• Provide comprehensive OT evaluations based on physician orders.
• Develop and implement treatment plans with long- and short-term goals, frequency, duration, and modalities.
• Document progress, treatment plans, discharges, and required facility forms in a timely manner.
• Ensure orders are obtained for evaluations, treatments, recertifications, and discharges.
• Supervise junior staff, students, and technicians as needed and participate in team meetings.
