Job summary

Waverly Rehabilitation And Healthcare Center in Waverly, VA is seeking an Occupational Therapist to join our long-term care team. This full-time/PRN/part-time role offers a flexible schedule and the opportunity to provide quality care to residents under MD orders.

The ideal candidate holds an OT degree and an active license, with SNF experience preferred. You will conduct evaluations, develop treatment plans, document progress, and collaborate with the interdisciplinary team to promote patient

Qualifications

• SNF experience preferred.
• Holds current license and/or registration in the state.

Responsibilities

• Provide comprehensive OT evaluation based on MD orders.
• Document findings on standardized evaluation forms timely and accurately.
• Prepare comprehensive treatment plan with long- and short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver comprehensive treatment using modalities and adjust as needed.
• Document daily participation and progress timely.
• Update treatment plan monthly for recertification.
• Document discharge summaries as indicated.
• Complete documentation forms required by the facility.
• Attend rehab meetings and participate in committees.
• Supervise junior staff and co-sign notes as indicated.
