Job summary

Waverly Rehabilitation And Healthcare Center in Waverly, VA is seeking a dedicated Occupational Therapist to join our long-term care team. You will assess residents, develop comprehensive treatment plans, and provide therapy across modalities in collaboration with an interdisciplinary staff.

The position emphasizes thorough documentation, adherence to MD orders, and ongoing care planning. A state license for OT is required, and SNF experience is preferred.

Qualifications

• Graduate of an accredited university with a degree in Occupational Therapy (B.S./M.A./M.S. or Doctorate) recognized by AOTA.
• SNF experience preferred.
• Current license/registration in the state as applicable.

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely, accurately and adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated while adhering to precautions.
• Document patient’s daily participation and progress on progress notes timely.
• Document monthly updated treatment plan on recertification comprehensively and in a timely manner.
• Document discharge on a discharge summary as indicated.
• Complete other documentation forms required by Facility.
• Document care plans and information needed for the MDS as deemed necessary by the facility.
• Attend facility and rehab meetings as indicated by Supervisor/Regional Manager.
• Participate in facility committee meetings as indicated.
• Act as a rehab delegate to the interdisciplinary team.
