Job summary

Waverly Rehabilitation And Healthcare Center in Waverly, VA, is seeking an Occupational Therapist to evaluate and treat residents under MD orders. This role involves comprehensive assessments, treatment planning, documentation, and collaboration with the interdisciplinary team.

Candidates must have an accredited OT degree recognized by AOTA, be licensed in the state, and preferably SNF experience. The facility offers opportunities for professional growth and a supportive work environment.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy that is recognized by the AOTA.
• SNF experience preferred
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide comprehensive OT evaluations based on MD orders.
• Document findings on the evaluation form timely and accurately.
• Develop comprehensive treatment plans with goals, frequency and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Deliver treatment with appropriate modalities and modify as needed while adhering to precautions.
• Document daily participation and progress in progress notes.
• Prepare discharge summaries and update treatment plans for recertification.
• Co-sign notes and supervise junior staff as needed.
• Maintain professional relationships with staff, patients and families.

Education

B.S., MA, M.S., or Doctorate in Occupational Therapy (AOTA recognized)
