Job summary

Waverly Rehabilitation And Healthcare Center in Waverly, VA is seeking a full-time Occupational Therapist to join our care team. The OT will perform comprehensive evaluations, develop treatment plans, document progress, and collaborate with MD orders and the interdisciplinary staff to meet patient goals.

Qualifications

• Graduate of an accredited university with a B.S., MA, M.S., or Doctorate in Occupational Therapy recognized by AOTA.
• SNF experience preferred.
• Holds current OT license/registration in the state.

Responsibilities

• Provide comprehensive OT evaluations based on MD orders.
• Document findings on evaluation forms timely and accurately.
• Create treatment plans with long- and short-term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver comprehensive therapy using modalities and adjust as needed.
• Document daily patient participation and progress.
• Prepare monthly updated treatment plans for recertifications and discharge summaries as indicated.
• Co-sign notes and supervise junior staff as needed.
• Attend meetings and participate in facility activities and committees.
