Job summary

Seven Oaks Rehabilitation and Healthcare Center in Columbia, SC is hiring an Occupational Therapist to provide comprehensive OT evaluations and treatments for long-term care residents. You will develop treatment plans with measurable goals, document progress, and coordinate with the interdisciplinary team to ensure quality patient care.

The role requires current OT licensure, AOTA recognition, and preferred SNF experience. Flexible scheduling and a supportive team environment are emphasized.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• SNF experience preferred
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertification's and discharges.
• Document progress and plan updates, and discharge summaries as indicated.
• Supervise junior staff, students and technicians as needed.
