Job summary

Qualifications

• OT degree from an accredited program
• SNF experience preferred
• Current license/registration in NC (as applicable)

Responsibilities

• Provide comprehensive OT evaluation based on MD orders
• Document findings on standardized evaluation forms timely
• Develop comprehensive treatment plans with long/short-term goals and modalities
• Obtain MD orders for evaluations, treatments, recertifications, discharges
• Document progress and update treatment plans monthly
• Maintain patient confidentiality and comply with facility policies and HR rules
