Qualifications

Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
SNF experience preferred.
Holds current OT license in the state (as applicable).

Responsibilities

Provide a comprehensive occupational therapy evaluation based on MD orders.
Document findings on the standardized evaluation form timely and accurately.
Develop a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
Deliver comprehensive treatment using modalities and adjust plans as needed while following precautions.
Document daily participation, progress notes, and monthly updated treatment plans.
