Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is hiring an Occupational Therapist (OT) for full-time, part-time, or PRN roles. A $10,000 relocation bonus is offered to qualified candidates who relocate for this position.

Responsibilities include comprehensive OT evaluations, developing treatment plans, documenting progress, and collaborating with the interdisciplinary team to support resident well-being. Licensure in NC is required and SNF experience is preferred.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• SNF experience preferred
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive OT evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Develop a comprehensive treatment plan with long/short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver comprehensive treatment utilizing modalities and modify as indicated while adhering to precautions.
• Document daily participation and progress in progress notes promptly.
• Update treatment plan monthly on recertification comprehensively and timely.
• Document discharge summaries when indicated.
• Fill out in-service documentation for caregivers or discharge as indicated.
• Complete all facility-required documentation.
• Prepare care plans and information needed for the MDS as required.
• Attend facility and rehab meetings as directed by Supervisor/Regional Manager.
• Participate in facility committees as indicated.
• Act as a rehab delegate to the interdisciplinary team.
• Assist in scheduling of patients as indicated.
• Perform screens as indicated.
• Complete billing and bill patients accurately for OT services rendered.
• Cover other therapists' caseload during absences as necessary.
• Supervise junior staff, students and technicians; co-sign notes where indicated.
• Maintain professional relationships with staff, patients and families.
• Provide in-service education to rehab staff, facility staff, hospitals and communities as needed.
• Comply with all facility policies and patient confidentiality requirements.
• Report to work on time and maintain a professional image; adhere to productivity requirements.
• Comply with HR policies re: lunch, sick and time off notification.
• Maintain a positive work atmosphere and foster positive relations.
• Perform any other duties as assigned by supervisor or RM.
