Job summary

Napa Post Acute in Napa, CA is seeking a compassionate Occupational Therapist (OT) to join our rehab team. The role focuses on evaluating residents, developing individualized treatment plans, and helping patients regain independence in daily activities.

Qualifications

• Current California Occupational Therapy license required.
• Post-acute or skilled nursing experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services that improve function and daily living skills.
• Collaborate with PTs, STs, nursing, and interdisciplinary team members.
• Complete timely and accurate documentation.
• Educate residents, families, and staff on treatment goals and care strategies.
• Ensure compliance with all applicable regulatory and clinical standards.
