Job summary

Ahava Therapy Services is seeking an Occupational Therapist to join our team dedicated to maximizing patient potential. You will evaluate functional status, develop individualized plans, and document progress with a focus on evidence-based practices.

Collaborate with nursing and therapy teams to optimize outcomes, educate families, and contribute to continuum of care. Light on-site responsibilities include adherence to HIPAA and payer guidelines while maintaining productivity.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy.
• Active OT license in the state of practice.
• BLS/CPR certification (current or obtained during orientation).
• Strong clinical reasoning, communication, and documentation skills.
• Ability to collaborate effectively within an interdisciplinary team.

Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Educate residents, families, and caregivers about exercises and therapeutic procedures.
