Qualifications

• Active Texas Occupational Therapy license in good standing.
• SNF or long-term care experience preferred.
• Knowledge of current OT treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home and living environments; recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per established plan of care and monitor progress.
• Document thoroughly and timely according to policies and regulations.
• Collaborate with the interdisciplinary team to ensure high-quality care.
