Envision Unlimited in Illinois is seeking an Occupational Therapist to join an interdisciplinary team focused on transitioning individuals with mental and physical illnesses to community living. You will assess ADLs/IADLs, design treatment plans, and support independent living with meaningful occupations.

The role requires a Master’s in Occupational Therapy and IL licensure, with preference for mental health or community care experience, and a valid driver’s license.

Qualificações

• Master’s degree in Occupational Therapy and IL licensure required.
• Prefer experience in mental health or community settings.
• Valid driver's license required.
• Must pass background check.

Responsabilidades

• Provide compassionate care and recovery‑oriented services to Class Members.
• Evaluate ADLs, IADLs, and functional mobility.
• Assess variables influencing a Class Member’s abilities and transitions.
• Develop and implement treatment plans to support community transition.
• Assist independent living goals with skills training and education.
• Attend team meetings and collaborate with interdisciplinary professionals.
• Comply with Envision Unlimited, DHS, and IL OT Act guidelines.
