Job summary

Envision Unlimited in Tinley Park, IL seeks a Transition Therapist to join an interdisciplinary team. The role provides basic clinical services to individuals with mental illness or disabilities, supporting transitions from long-term care to community settings.

Responsibilities include counseling, crisis intervention, and coordination with treatment teams to inform families and agencies while maintaining confidentiality and timely documentation.

Qualifications

• Master’s degree in social work, counseling, psychology or related field with at least three years’ experience in social services.
• Must have excellent written and verbal communication skills, sufficient decision making, problem solving, computer and organizational skills.
• Individual must be able to work autonomously.

Responsibilities

• Conduct assessments.
• Provide on-site, supportive counseling to CM’s residing in nursing homes and SMRFS.
• Utilize motivational interviewing and other techniques to assist CM’s make decisions and long-term goals.
• Assist CMs identify crisis triggers, and contingency plans in the event a crisis arises post discharge from the facility.
• Maintain and update client’s pertinent information within the MH electronic care system.
• Achieve required productivity targets and complete documentation in a timely manner, including clinical documentation and service reporting.
• Inform supervisor of any condition that impacts the safety and wellbeing of clients served.
• Attend individual supervision with supervisor at scheduled time weekly to review and report on progress, productivity, and concerns.
