Continuum Therapy Partners in Illinois seeks an Occupational Therapist for a split position covering Salem, Aviston, Mt. Vernon facilities. The role focuses on improving patients' functional independence through individualized plans that you will develop and monitor.

Requirements include a Master’s or Doctorate in OT, an active state license, NBCOT certification, and ideally one year of OT experience. Full-time daytime schedule with a comprehensive benefits package is available.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of OT experience preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
