Job summary

Sage Connections in Elgin, IL is hiring a W-2 Occupational Therapist to provide K–12 school-based services for the 2026-2027 school year. You will deliver direct and consultative OT aligned with IEPs and collaborate with teachers and families to support student participation.

The role requires a Master’s in OT, NBCOT (OTR), IL license, and PEL with School Support Personnel endorsement (or eligibility). Schedule is Monday–Friday, 7:30am–3:30pm, 37.5 hours/week, onsite in Elgin, IL.

Qualifications

• Master’s degree in Occupational Therapy is required.
• NBCOT certification (OTR) is required.
• Current Illinois Occupational Therapy license (IDFPR) is required.
• Illinois Professional Educator License (PEL) with School Support Personnel endorsement in Occupational Therapy (or eligibility) is required.
• School-based experience is preferred but not required.

Responsibilities

• Provide direct and consultative occupational therapy services aligned with student IEPs.
• Conduct evaluations and assessments to determine OT needs.
• Develop individualized treatment plans and goals.
• Collaborate with teachers, families, and school staff to support student participation.
• Document services and progress accurately and in a timely manner.
• Participate in IEP meetings and contribute to educational planning.
