Job summary

Sage Connections, LLC is hiring a full-time W-2 Occupational Therapist to deliver K–12 school-based OT services for the 2026–2027 school year. The role supports students across districts with a collaborative team, aligned with IEPs, and a focus on practical outcomes in classroom settings.

Onsite in Illinois, the schedule runs Monday–Friday, 8:00AM–3:30PM, with 35–40 hours per week, August 2025 through May 2026. We value therapists who are passionate about diverse learners and professional growth.

Qualifications

• Master’s degree in Occupational Therapy.
• Current Illinois License in Occupational Therapy.
• School-based experience preferred, but not required.
• Strong communication and collaboration skills.
• Passion for supporting diverse learners in a school setting.

Responsibilities

• Provide direct and consultative occupational therapy services aligned with student IEPs.
• Conduct evaluations and assessments to determine OT needs.
• Develop individualized treatment plans and goals.
• Collaborate with teachers, families, and school staff to support student participation.
• Document services and progress accurately and in a timely manner.
• Participate in IEP meetings and contribute to educational planning.
