Job summary

Sage Connections is seeking a full-time W-2 Certified Occupational Therapist Assistant (COTA) to provide school-based services for K–12 students in Illinois for the 2025–2026 school year. The role works under supervision, collaborating with teachers, families, and staff to support student participation and progress.

Onsite in Illinois with a Monday–Friday schedule, 8:00 AM–3:30 PM, totaling 35–40 hours weekly. We value growth, teamwork, and empowering specialists in our mission-driven company.

Qualifications

• Associate's Degree (or higher) in Occupational Therapy.
• Current Illinois Occupational Therapy Assistant (COTA) license.
• School-based experience preferred, but not required.
• Strong communication and collaboration skills.
• Passion for supporting diverse learners in a school setting.

Responsibilities

• Provide direct occupational therapy services under the supervision of a licensed Occupational Therapist, aligned with student IEPs.
• Implement individualized treatment plans and goals developed by the supervising OT.
• Collaborate with teachers, families, and school staff to support student participation.
• Monitor student progress and communicate updates to the supervising OT.
• Document services and progress accurately and in a timely manner.
• Support IEP meetings and educational planning as appropriate.
