Job summary

Sage Connections is seeking a dedicated Occupational Therapist to provide K–12 school-based services for the 2026–2027 school year. The role is onsite at multiple districts in Oregon, with a Monday–Friday schedule from 8:00 AM to 3:30 PM and 37.5–40 hours per week.

Qualifications

• Master’s degree in Occupational Therapy.
• Current Oregon License in Occupational Therapy.
• School-based experience preferred, but not required.
• Strong communication and collaboration skills.
• Passion for supporting diverse learners in a school setting.

Responsibilities

• Provide direct and consultative occupational therapy services aligned with student IEPs.
• Conduct evaluations and assessments to determine OT needs.
• Develop individualized treatment plans and goals.
• Collaborate with teachers, families, and school staff to support student participation.
• Document services and progress accurately and in a timely manner.
• Participate in IEP meetings and contribute to educational planning.
