Job summary

Americare Inc. is seeking an Independent Contractor Occupational Therapist (OT) to deliver home-based rehabilitation services in Wisconsin.

You will follow physician-prescribed plans to help patients regain daily living skills and independence, updating care plans as needed and coordinating with the care team to ensure high-quality rehab at home. Two years of OT experience, WI licensure, and ACOTE-approved education are preferred.

Qualifications

• Must be a licensed Occupational Therapist in Wisconsin (WI).
• Two+ years of experience in home health or outpatient OT preferred.
• Ability to work independently and follow physician-prescribed plans.

Responsibilities

• Provide OT services to patients in their homes per physician orders.
• Update and revise care plans, document progress, and coordinate with care team.
• Deliver compassionate, one-on-one patient care and promote independence.
• Maintain professional standards and comply with licensing requirements.
