Job summary

Phyt Rehab seeks a licensed Speech Language Pathologist to provide home-based therapy as an independent contractor in Wisconsin. You will follow physician-prescribed plans, update care plans, and communicate with the care team to deliver coordinated rehab services in patients' homes.

The role requires a master’s degree in Speech-Language Pathology and a minimum of one year supervised experience, with ASHA membership and certification preferred. Flexibility and independent judgment are valued.

Qualifications

• A licensed Speech Language Pathologist by the State of Wisconsin.
• A master’s degree is required and one full year of supervised work experience.
• Speech Language Pathologist supervisors must have at least three (3) years of clinical experience.
• The Speech Language Pathologist is a member of the American Speech and Hearing Association and is certified by the association.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Help patients regain the skills they need for daily living and independence.
• Follow physician-prescribed plans to reduce physical limitations, update and revise care plans as needed.
• Communicate with the care team to ensure coordinated, top-quality rehab services.

Position Details

Independent Contractor - Speech Language Pathologist (SLP) As an Independent ContractorOccupational Therapist with Phyt Rehab, you’ll help patients regain the skills they need for daily living and independence. You’ll follow physician-prescribed plans to reduce physical limitations, update and revise care plans as needed, and communicate with the care team to ensure coordinated, top-quality rehab services.
