Your Role & Responsibilities

As a Home Health OT, you will provide skilled occupational therapy services to a diverse caseload of clients with various conditions, including:

  • Neurological, musculoskeletal, orthopedic, and cardiopulmonary impairments

  • Fine motor, sensory-motor, and developmental delays

  • Traumatic injuries and swallowing disorders

Key responsibilities include:

  • Conducting comprehensive initial evaluations and developing individualized, evidence-based Plans of Care.

  • Providing direct therapeutic interventions to improve clients' safety, independence, and functional abilities.

  • Educating clients, family members, and caregivers on therapeutic techniques, home exercise programs, and adaptive equipment.

  • Completing timely and accurate documentation electronically.

  • Collaborating with physicians, nurses, and other members of the interdisciplinary healthcare team.

  • Managing discharge planning to ensure a safe transition for the client.

Qualifications of the Ideal Candidate

We are looking for a dependable, energetic, and empathetic professional with excellent clinical and interpersonal skills.

Required:

  • Valid, current, and unrestricted Occupational Therapist license in the State of Wisconsin.

  • A professional, positive, and patient-centered attitude.

  • Strong organizational, assessment, and problem-solving skills.

  • Excellent communication skills for interacting with patients, families, and physicians.

  • Proficiency with technology and ability to chart electronically.

  • A valid driver’s license, reliable vehicle, and current automobile insurance.

  • Ability to pass a pre-employment physical, drug screen, and background check.

  • Current TB skin test or willingness to complete one upon hire.

Preferred:

  • 1-3 years of experience in a home health setting is a plus, but not required. We welcome motivated therapists with relevant clinical experience to apply.
