Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our growing team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and work with facility staff to ensure excellent patient outcomes.

In this role you will supervise Occupational Therapy Assistants, document evaluations and progress, and contribute to the development of measurable functional goals while delivering compassionate care to patients and families.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
