Concept Therapy PC & Concept Therapy Home Health in Indiana seeks an Occupational Therapist to deliver outpatient and in-home services. You will assess functional needs, design customized plans, and collaborate with physicians, nurses, therapists, and caregivers to support meaningful outcomes.

We are expanding and looking for a compassionate, motivated Occupational Therapist to join our team. This role includes opportunities in both outpatient therapy and skilled home health, allowing you to practice across settings and support patients throughout their full continuum of care.
