Job summary

Concept Rehab is seeking a compassionate Occupational Therapist to join our skilled nursing and post-acute care team in Cincinnati, OH. The role offers Full-Time & PRN opportunities to help patients regain independence in daily activities and functional mobility.

You will perform evaluations, develop treatment plans, and deliver ADL/IADL interventions while collaborating with nurses, physicians, and other therapists.

Qualifications

• Degree in Occupational Therapy (Bachelor/Master/Doctorate).
• Current Ohio OT license in good standing.
• Experience in skilled nursing or post-acute rehab preferred; new graduates welcome.

Responsibilities

• Perform comprehensive OT evaluations and functional assessments.
• Develop individualized treatment plans based on patient goals.
• Deliver interventions focusing on ADLs/IADLs.
• Address cognitive, perceptual, sensory, and mobility deficits.
• Recommend adaptive equipment and home safety strategies.
• Educate patients, families, and caregivers to support outcomes.
• Collaborate with nursing, physicians, and rehab staff.
• Supervise COTAs as appropriate.
• Participate in care plan meetings and discharge planning.
• Maintain accurate documentation per regulations.

Requirements

What You'll Do

• Perform comprehensive occupational therapy evaluations and functional assessments
• Develop individualized treatment plans based on patient goals and clinical needs
• Deliver skilled interventions focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Address cognitive, perceptual, sensory, upper extremity, and functional mobility deficits
• Recommend adaptive equipment, environmental modifications, and safety strategies
• Educate patients, families, and caregivers to support successful outcomes
• Collaborate with nursing, physicians, rehabilitation professionals, and facility leadership
• Supervise and support Certified Occupational Therapy Assistants (COTAs) as appropriate
• Participate in interdisciplinary care plan meetings and discharge planning discussions
• Maintain accurate, timely, and compliant documentation in accordance with regulatory requirements

Qualifications

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy
• Current Ohio Occupational Therapist license in good standing
• Skilled nursing, geriatric, or post-acute rehabilitation experience preferred
• Strong clinical reasoning and problem‑solving abilities
• Excellent communication and interpersonal skills
• Commitment to patient-centered care and positive clinical outcomes
