Concept Rehab is seeking a compassionate Occupational Therapist to join our rehabilitation team in skilled nursing and post-acute care settings in Cincinnati, OH. You will evaluate patients, develop tailored treatment plans, and deliver ADL/IADL therapies to improve independence and quality of life.

The role emphasizes collaboration with nursing and medical staff, education for patients and families, and supervision of COTAs as appropriate.

Qualifications

• Licensed OT with degree in Occupational Therapy.
• Current Ohio OT license in good standing.
• New graduates encouraged to apply.
• Experience in skilled nursing or post-acute rehabilitation preferred

Responsibilities

• Perform comprehensive occupational therapy evaluations and functional assessments
• Develop individualized treatment plans based on patient goals and clinical needs
• Deliver skilled interventions focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Address cognitive, perceptual, sensory, upper extremity, and functional mobility deficits
• Recommend adaptive equipment, environmental modifications, and safety strategies
• Educate patients, families, and caregivers to support successful outcomes
• Collaborate with nursing, physicians, rehabilitation professionals, and facility leadership
• Supervise and support Certified Occupational Therapy Assistants (COTAs) as appropriate
• Participate in interdisciplinary care plan meetings and discharge planning discussions
• Maintain accurate, timely, and compliant documentation in accordance with regulatory requirements
