Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care, collaborating with facility staff to ensure exceptional patient outcomes.

You will provide ongoing patient education, re-evaluate progress, and maintain timely documentation while supervising Occupational Therapy Assistants to ensure consistent, high-quality care.

Qualifications

• Experience in SNF or geriatric settings
• Ability to develop and monitor plans of care
• Strong communication with patients and families

Responsibilities

• Evaluate and treat patients following an individualized plan of care

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
