The Lodge at Heritage Grove Estates (SNF) in Topeka, KS is hiring a full-time Occupational Therapist to assess, plan, and implement individualized therapy programs for our residents. Qualifications

• Initial and ongoing assessments of residents' physical, cognitive, and emotional abilities and develop individualized treatment plans to achieve identified goals

• Collaborate with interdisciplinary team members to ensure comprehensive care and address resident needs
• Educate residents, families, and caregivers on adaptive techniques, safety measures, and strategies to promote independence
• Document resident progress, goals, and treatment plans in accordance with facility policies and regulatory requirements
