Job summary

The Lodge at Heritage Grove Estates (SNF) in Topeka, KS is hiring a full-time Occupational Therapist to assess, plan, and implement individualized therapy programs for our residents. You will help restore independence, mobility, and cognitive function while collaborating with the therapy team to deliver compassionate care.

Requirements include a degree in OT and current Kansas licensure; the role offers a $5,000 sign-on bonus and competitive benefits.

Qualifications

• Initial and ongoing assessments of residents' physical, cognitive, and emotional abilities and develop individualized treatment plans to achieve identified goals
• Implement and modify occupational therapy programs to promote independence, mobility, and cognitive function
• Collaborate with interdisciplinary team members to ensure comprehensive care and address resident needs
• Educate residents, families, and caregivers on adaptive techniques, safety measures, and strategies to promote independence
• Document resident progress, goals, and treatment plans in accordance with facility policies and regulatory requirements

Responsibilities

• Conduct assessments and create individualized OT plans.
• Deliver and adjust therapy interventions to improve function and independence.
• Collaborate with nurses, physicians, and therapy staff.
• Educate families and caregivers on home strategies and safety.
