Job summary

Northern Cardinal Rehabilitation and Nursing in Suffolk, VA is seeking an Occupational Therapist (OT) to join our resident-centered team. You will assess referred patients, develop and manage treatment plans, and collaborate with a multidisciplinary staff to deliver high-quality, patient-focused care.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must hold a valid professional license and registration for state of practice.
• Experience with long-term care or geriatric care is preferred.

Responsibilities

• Assess referred patients for rehabilitation needs and plan treatments.
• Provide direct patient care to assess medical condition, function, and rehab potential.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Set realistic, achievable goals for patients and document progress.
• Record patients’ condition and progress in charts and notes.
• Educate patients and families on therapy methods and goals.
• Perform evaluations, weekly progress notes, re-certifications and discharge docs.
• Collaborate with all disciplines to ensure patient-centered care.
• Carry out additional duties as directed by the Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services




• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.




• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.




• Set realistic and achievable goals for their patients




• Document and record the patients' condition




• Educate patients and families in an appropriate therapy method.




• Evaluations, weekly progress notes, re-certification and discharge documentations.




• Collaborates with all disciplines to provide best patient centered treatment and care.




• Perform all other duties assigned by Director of Rehabilitation




• Compliance with confidentiality, HIPPA and Federal Residents Rights.
