Job summary

Premier Healthcare LLC is seeking an Occupational Therapist (OT) to join our family of skilled nursing facilities and rehabilitation centers in New York. The role focuses on resident-centered, quality care, evaluating residents and developing personalized treatment plans to promote independence in daily activities.

You'll collaborate with an in-house therapy team to implement interventions, document progress, and educate families on home programs, while adhering to HIPAA and Residents' Rights

Qualifications

  • Graduate of an accredited Occupational Therapy program.
  • Valid OT license and registration for the state of practice.
  • Experience in long-term care or geriatrics is preferred but not required.
  • Strong communication and organizational skills.
  • Compassionate, team-oriented, and committed to resident-centered care.

Responsibilities

  • Evaluate referred residents to determine functional level and rehabilitation potential.
  • Provide direct care and develop individualized treatment plans to regain independence in ADLs.
  • Establish goals based on residents' abilities and physician recommendations.
  • Implement interventions to improve strength, mobility, cognition, and self-care.
  • Educate residents and families on home programs and adaptive techniques.
  • Complete documentation including evaluations, progress notes, recertifications, and discharge summaries.
  • Collaborate with the interdisciplinary team to ensure resident-centered care.
  • Adhere to regulations including HIPAA and Residents' Rights.
  • Perform additional duties as assigned by the Director of Rehabilitation.

Duties and Responsibilities:

  • Perform additional duties as assigned by the Director of Rehabilitation.

Requirements/Qualifications:
