Job summary

Eastern Health Group seeks an Occupational Therapist (OT) to assess and treat residents in our skilled nursing and rehabilitation centers. You will develop treatment plans, document progress, and work with a multidisciplinary team to deliver patient-centered care.

Must have an OT degree and current state license; experience in long-term or geriatric care is preferred. Join a facility focused on resident-centered care and quality outcomes.

Qualifications

• Graduated from an accredited OT program.
• Current license and registration for state of practice.
• Experience with long-term care or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to evaluate medical condition and functional ability.
• Establish and administer a treatment plan with goals per patient capacity.
• Document patient condition and progress.
• Educate patients and families on therapy methods.
• Perform evaluations, progress notes, re-certifications, and discharge documents.
• Collaborate with disciplines to provide patient-centered care.
• Perform duties as assigned by Director of Rehabilitation.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients' condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
