Job summary

Northern Cardinal Rehabilitation and Nursing is seeking a Physical Therapist (PT) to deliver skilled nursing rehab, assess referrals and lead treatment programs that improve function and safety for patients in our care setting.

You will document progress, educate patients and families, and collaborate with the care team to achieve excellent outcomes while adhering to privacy regulations. We offer engagement activities and competitive pay and benefits.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Licensed and registered to practice in the state.
• Experience in long-term/geriatric care preferred.
• Familiarity with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care and assess medical condition and functional capacity.
• Establish and administer a treatment program with goals per patient capacity.
• Set realistic goals and document patient status and education for families.
• Complete evaluations, progress notes, recertifications and discharge documents.
• Collaborate with disciplines to deliver patient-centered care.
• Perform duties as directed by Director of Rehabilitation.
• Ensure confidentiality and HIPAA compliance.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Requirements/Qualifications:

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
