Job summary

Eastern Health Group is seeking an Occupational Therapist (OT) to join our resident-centered rehabilitation team in Newport News, VA. You will assess referred patients, provide direct care, and develop treatment plans under physician direction to improve functional outcomes.

Responsibilities include evaluations, progress notes, documentation, family education, and collaboration with all disciplines to ensure patient-centered care. HIPAA compliance and rights protection are required.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid license and state registration.
• Experience with long-term or geriatric care is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition and rehabilitation potential.
• Establish and administer a treatment program with goals set by the physician.
• Set realistic and achievable goals for patients.
• Document and record patient conditions.
• Educate patients and families in therapy methods.
• Perform evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborate with all disciplines for patient-centered care.
• Perform other duties as assigned by Director of Rehabilitation.
• Maintain confidentiality and residents' rights (HIPAA).

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services




• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.




• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.




• Set realistic and achievable goals for their patients




• Document and record the patients' condition




• Educate patients and families in an appropriate therapy method.




• Evaluations, weekly progress notes, re-certification and discharge documentations.




• Collaborates with all disciplines to provide best patient centered treatment and care.




• Perform all other duties assigned by Director of Rehabilitation




• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.




• Must have a valid professional license and registration for state of practice.




• Experience with long-term care and/or geriatric care is preferred but not limited to.
