Concept Rehab is seeking a compassionate Occupational Therapist to join our skilled nursing and post-acute care team in Cincinnati, OH. You will evaluate and treat patients to improve daily living activities and independence.

This full-time position includes PRN options, competitive compensation, and a supportive, therapist-led culture focused on outcomes and ongoing professional development.

Qualifications

• Current Ohio Occupational Therapist license in good standing.
• Degree in Occupational Therapy (Bachelors/Masters/Doctorate).
• Geriatric/post-acute rehab experience preferred.
• New graduates encouraged to apply.

Responsibilities

• Perform comprehensive occupational therapy evaluations and functional assessments.
• Develop individualized treatment plans based on patient goals and clinical needs.
• Deliver interventions focused on ADLs and IADLs.
• Address cognitive, perceptual, sensory, upper extremity, and mobility deficits.
• Suggest adaptive equipment and environmental modifications.
• Educate patients, families, and caregivers to support outcomes.
• Collaborate with nursing, physicians, and rehabilitation staff.
• Supervise and support COTAs as appropriate.
• Participate in care planning and discharge discussions.
• Maintain accurate, timely documentation per regulatory requirements.

Requirements

What You\'ll Do

• Perform comprehensive occupational therapy evaluations and functional assessments
• Develop individualized treatment plans based on patient goals and clinical needs
• Deliver skilled interventions focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Address cognitive, perceptual, sensory, upper extremity, and functional mobility deficits
• Recommend adaptive equipment, environmental modifications, and safety strategies
• Educate patients, families, and caregivers to support successful outcomes
• Collaborate with nursing, physicians, rehabilitation professionals, and facility leadership
• Supervise and support Certified Occupational Therapy Assistants (COTAs) as appropriate
• Participate in interdisciplinary care plan meetings and discharge planning discussions
• Maintain accurate, timely, and compliant documentation in accordance with regulatory requirements

Qualifications

• Bachelor\'s, Master\'s, or Doctorate degree in Occupational Therapy
• Current Ohio Occupational Therapist license in good standing
• Skilled nursing, geriatric, or post-acute rehabilitation experience preferred
• Strong clinical reasoning and problem-solving abilities
• Excellent communication and interpersonal skills
• Commitment to patient-centered care and positive clinical outcomes
