Job summary

Santana Hills Healthcare Center in San Jose, CA is seeking an Occupational Therapist to evaluate, plan, and implement individualized treatment programs to help patients develop, recover, and improve daily living skills. New graduates are welcome to apply in a fast‑paced rehab environment within a 167‑bed facility offering skilled nursing and sub‑acute care.

You will work with a multidisciplinary team, ensure accurate documentation, and educate patients and families on therapy techniques.

Qualifications

• Bachelor’s or Master’s degree in Occupational Therapy.
• Valid Occupational Therapist license/certification.
• Strong communication and interpersonal skills.

Responsibilities

• Assess patients’ physical, cognitive, and emotional abilities.
• Develop personalized treatment plans and therapy goals.
• Provide therapeutic interventions to improve independence and quality of life.
• Educate patients and families on therapy techniques and adaptive equipment.
• Maintain accurate patient records and progress notes.
• Collaborate with physicians, nurses, caregivers, and other healthcare professionals.
• Monitor patient progress and modify treatment plans as needed.
• Ensure compliance with healthcare regulations and safety standards.

We are seeking a compassionate and motivated Occupational Therapist to join our healthcare team. The Occupational Therapist will evaluate, plan, and implement individualized treatment programs to help patients develop, recover, and improve the skills needed for daily living and working

Key Responsibilities

• Assess patients’ physical, cognitive, and emotional abilities
• Develop personalized treatment plans and therapy goals
• Provide therapeutic interventions to improve independence and quality of life
• Educate patients and families on therapy techniques and adaptive equipment

What We Are Looking For
